'2007 Tri-City Indoor "Turkey" Tournament
Dear Coach, Registration forms and checks are due by the end of day Saturday, November 3. The registration fee is $12 per player. A minimum of six players is needed to register. Please mail or deliver this completed form and your SINGLE CHECK payable to AYSO INDOOR SOCCER for the entry fee ($12 per player) to the tournament director in your region at the address on the form. If you mail your registration form and check, please allow enough time for delivery. U14, U12, and U10 age groups will be competitive with playoffs (and trophies awarded to age group winners) on Sunday. U8 will be played on a non-competitive basis. All divisions will have boys and girls teams. All players will receive a commemorative pin.
Additional players may be added after the team registration has been submitted but must be done prior to your first game. All players must be from your regular Fall AYSO team. The only exception to this rule is that U8 teams with fewer than six participating players may add players from other AYSO U8 teams. The $12 fee for each additional player must be paid before the team’s first game. Please bring your AYSO registration forms with you at all times during the tournament, as they are required for emergency treatment and proof of insurance.
